Athlete Insurance Claim:

Parents, if one of your children, on your active roster is PHYSICALLY HURT while at an official TYFA event, you may attempt to make a claim. Please remember TYFA's insurance is a Secondary Insurance; not a Primary unless you do not have insurance of your own.

Does the athlete qualify (all must be true)

1. Is the athlete's organization "eligible" for the season they wish to claim?

2. Is the athlete "eligible" for the season the parents wish to claim? 

3. Did the injury occur during an official TYFA event?

If the answer to all the questions above is YES, you can continue.

STEP 1 - 

1. Download the AIG Medical Claim Form. 

2. Fill in all the empty fields highlighted YELLOW.

3. Email the completed Claim form to HelpDesk@tyfa.com.

STEP 2

1. TYFA will complete the claim form and send it to the insurance company for review. 

2. TYFA will CC the parent as well.

3. Once the parent receives the copy from TYFA, they should wait a minimum of 5  business days and then contact the insurance company to set up the Claim Number.
